We’re Introducing A New Electronic Medical Records (EMR) System

We are introducing a new Electronic Medical Record (EMR) system.

There may be some delays in your appointment time as we are transitioning.

We ask for your patience and flexibility as we update your patient information to ensure accuracy as we continue the transition to our new EMR system.

When you arrive for your appointments, please be prepared to show or bring the following:

  1. Photo identification
  2. All health insurance cards (we do require copies for your chart)
  3. A list of all medications you are currently taking, including vitamins and over the counter medications
  4. Any co-payment amounts you are aware of through your insurance
